Occlusal Cant Review Flags Molar Compensation Needing Presurgical Decompensation

Patients with skeletal Class II or Class III asymmetry and occlusal cant showed significant buccal tipping of maxillary molars on the deviated side, a compensation pattern that calls for specific presurgical decompensation to prevent relapse, according to a systematic review published Sept. 28 in BMC Oral Health.

The review, registered in PROSPERO, included 23 observational studies of patients with skeletal Class II or Class III malocclusion and facial asymmetry complicated by occlusal cant. Of those studies, eight were rated good quality, 10 moderate, and five poor.

Three-dimensional imaging was identified as essential for distinguishing transverse from vertical asymmetry. The authors wrote that 3D imaging has redefined the cause of occlusal cant as primarily a manifestation of vertical ramal asymmetry.

On surgical management, isolated mandibular surgery was effective for moderate lip-line cants ranging from 1.5 to 6 degrees, with soft tissue outcomes comparable to two-jaw surgery. Severe vertical discrepancies required differential maxillary impaction.

The authors proposed a visual threshold of 4 degrees as a clinical guide for determining whether surgical intervention is needed.

Neha Shani, Prajwal Shetty, Ravi M S, and Shilpa M of the A B Shetty Memorial Institute of Dental Sciences at NITTE (Deemed to be University) in Mangalore, India, conducted the review. It received no specific funding, and the authors declared no competing interests.

All included studies were observational, and fewer than half were rated good quality, which limits how firmly the proposed 4-degree threshold can be applied as a treatment rule.
