Nestled on the island of Guam, Future Smiles Orthodontics lives up to its name. Led by Dr. Paul Sablan Sauget, a board-certified orthodontist with a passion for innovation, the practice blends advanced technology, personalized patient care, and a forward-thinking approach to create an experience that feels anything but traditional. From early orthodontic intervention to complex case management, “Dr. Paul” and his team are redefining what it means to deliver orthodontics in a modern, patient-centered setting.


A journey rooted in passion and precision
Dr. Paul’s path to orthodontics began with hands-on experience. He worked as a dental and orthodontic assistant throughout college for several orthodontists in Guam (including his older brother, also an orthodontist) and in Milwaukee, Wisconsin, while studying biomedical sciences and business administration at Marquette University. He also competed on Marquette’s Division I tennis team.

After earning his DDS degree with honors from the University of the Pacific, he served as a dentist in the U.S. Marine Corps and Navy. Those years provided extensive training across all areas of dentistry. Still, it was the biomechanics and physics of tooth movement, as well as the life-changing impact on patients, that ultimately drew him to orthodontics. He then completed his orthodontics residency at Indiana University after serving on active duty.

“I loved the combination of precision and artistry,” he says. “Restoring function and creating confident smiles felt like the perfect balance of science and health care.”


Building future smiles
Before returning home to Guam, Dr. Paul successfully launched two orthodontic startups in California: SoCo Dental Specialty Group in Sonoma County and PCH Smiles Dentistry and Orthodontics in Los Angeles, while associating with Dr. David Alpan in Beverly Hills. In 2018, he opened Future Smiles Orthodontics, his third practice from the ground up, with a vision of combining advanced technology, education, and personalized patient care.

Today, his schedule balances three and a half days of patient care with dedicated administrative time for running the practice. “We structure the day intentionally,” he says. “Bigger procedures in the morning, lighter appointments in the afternoon. That rhythm helps maintain efficiency and quality for every patient.”

Robotics at the heart of the patient experience
Technology has always played a central role in Future Smiles Orthodontics, but the integration of Zeeno Robotics has elevated both patient education and the overall experience in ways that feel distinctly futuristic.

Their Zeeno robot, affectionately named “Stella,” serves as a robotic assistant, greeting patients, guiding them to different areas of the clinic, and even engaging children in interactive conversations to ease dental anxiety. One of Stella’s biggest contributions comes during consultations, when she presents educational videos on orthodontic appliances and treatment options.

“Zeeno allows patients to visualize complex procedures in real time,” Dr. Paul explains. “When they see how a palate expander works or what’s involved in placing a Class II corrector, they understand the process so much better. It has improved patient motivation and compliance while saving our team valuable chairside time.”

From escorting patients to showcasing treatment simulations, Zeeno has transformed the way Future Smiles communicates and connects with patients. Families often comment on how “futuristic” the experience feels, a reflection of the practice’s commitment to pairing cutting-edge tools with compassionate care.


Harnessing technology to improve outcomes
Beyond robotics, the practice leverages CBCT imaging and 3D printing to plan and execute treatment with greater precision. Dr. Paul has been an early adopter of CBCT technology since the mid-2000s, using it to identify impacted teeth, guide palatal expansion, and evaluate skeletal discrepancies in three dimensions.

“CBCT allows us to make more informed decisions,” he says. “It helps us recover teeth that might otherwise be compromised and improves long-term outcomes.”

The team’s in-house 3D printing capabilities support everything from study models to retainers and custom appliances. These tools are not about replacing clinical judgment, but rather about enhancing it. “Technology is a partner in care, not a shortcut,” he says. “It allows us to create better experiences and better results for our patients.”


A coaching mindset
Dr. Paul’s leadership philosophy has been shaped by his diverse background as a Division I athlete, U.S. military officer, and business owner. He believes in leading as a coach rather than a boss, fostering an environment where every team member feels empowered to contribute.

“Coaches guide, encourage, and motivate,” he says. “Bosses intimidate and command. I’ve seen firsthand that when you support your team and invest in their growth, they give their best to patients and to each other.”

That collaborative approach has resulted in a tightly knit, highly skilled team that patients come to know and trust. “An experienced, engaged staff is invaluable,” he adds. “Everyone here owns their role and takes pride in the care we deliver.”


Tailoring treatment to every patient
His office takes a personalized approach to care, whether patients need functional appliances, traditional braces, or aligners. Dr. Paul stresses the importance of early evaluation, screening children by age seven to identify potential problems before they become complex.

“Not every case is the same,” he says. “We consider growth patterns, patient maturity, and individual preferences before making recommendations. Offering multiple treatment options helps patients and families feel informed and involved.”


A memorable case
Among his many challenging cases, one stands out: a 12-year-old patient with a Class III malocclusion and two impacted canines. By carefully applying biomechanical principles, including the use of an RPE J-hook to reposition the canine crown, Dr. Paul successfully preserved both teeth and achieved a Class I finish.

“It was rewarding to see the outcome and the family’s response,” he recalls. “Cases like that remind you why attention to detail and early intervention matter so much.”


Teaching, mentorship, and giving back
In addition to leading Future Smiles Orthodontics, Dr. Paul dedicates time to teaching dental students, orthodontic residents, and interns. Whether through lectures, study clubs, or one-on-one mentorship, he views education as a way to strengthen the profession as a whole.

“Every time I teach, I learn something new,” he says. “Helping future orthodontists refine their skills and think critically about treatment makes all of us better clinicians.”


Advice for the next generation
For young orthodontists starting their first private practice, he offers one piece of advice: start small, stay focused, and grow at your own pace.

“You don’t need a massive patient pool to build a successful practice,” Dr. Paul says. “Keep overhead manageable, deliver quality care, and take it one patient at a time. If you stay true to your vision, the growth will come.”


Looking ahead
As Dr. Paul’s office continues to grow, he remains committed to pairing evidence-based care with innovative tools like Zeeno Robotics, CBCT, and 3D printing. He hopes to keep mentoring colleagues, sharing his expertise through lectures, and supporting the next generation of orthodontists.
